BAYS Community Care
BAYS Florida Inc., The City of Jacksonville (in partnership with the Jacksonville System of Care Initiative), The Partnership for Child Health, The Jacksonville Children’s Commission and Gateway Community Services has developed and implemented a Comprehensive Care Coordination system for youth who reside in Duval County with substance abuse and/or mental health issues entering the Juvenile Assessment Center (JAC).
Utilizing an evidence-based assessment instrument, this care coordination effort identifies youth entering the JAC, with an emphasis on first time offenders, for substance abuse and/or mental health issues. Specially trained care coordinators work to engage adolescents and their families through mitigating barriers that impede access to treatment and successful outcomes. The care coordinators work to ensure identified youth are referred to appropriate substance abuse and /or mental health services that are conveniently located.
As a result BAYS Community Care was launched in February 2015 as a partner to provide individual and group counseling to adolescents and their families residing in Duval County. Our goal is to provide quality counseling services that will impact young men and women in a positive way and assist the City of Jacksonville in addressing the substance abuse and mental health concerns within the community.
